Pre-Registration For COVID-19 Testing

You will get a QR code after you finish the registration process. Please show the on-site staff the QR code for them to start your visit.

If you are experiencing severe symptoms such as extreme shortness of breath, contact your physician. If it is an emergency, dial 911.

You do not need an appointment and no pre-registration is required but is encouraged to save you time.
Upon arrival, our staff will scan your pre-registration QR code.
For drive-ups, you will be asked to register on our patient portal, Intend/DragonflyPHD. Our staff is there to assist in the registration process.
Intend/DragonflyPHD is a HIPAA-compliant platform that is used to electronically deliver your test results securely and privately.
You do not need a prescription or referral from your doctor to get tested.
Please have your I.D. / driver’s license or Passport and insurance information (if you have insurance) ready when it’s your turn.

Your test should be covered by your insurance with no out-of-pocket costs. Please bring your health insurance card.
Self-pay options are available for the uninsured through credit/debit card only.

Remain in your car, lower your window 6 inches and our medical staff will collect your sample by nasal swab.
The testing process will take approximately 10 minutes.
Results will be delivered electronically to you as soon as they are available via Intend/DragonflyPHD.
